These Grant Application Terms & Conditions govern all applications for financial assistance submitted to The Shebie Foundation (“the Foundation”, “we”, “our”, “us”).
By submitting a grant application, applicants agree to the terms outlined below.
1. Purpose of Funding
The Shebie Foundation exists to help reduce financial barriers for women living with Endometriosis and Adenomyosis who require surgical treatment.
Financial assistance provided by the Foundation is intended to help eligible applicants with out-of-pocket medical costs associated with recommended surgical treatment.
Funding is limited and assistance is provided at the discretion of the Foundation.
2. Eligibility Requirements
To be considered for financial assistance, applicants must meet the following criteria:
Provide evidence of a confirmed diagnosis of Endometriosis from a qualified medical professional.
Provide documentation showing that surgical treatment has been recommended by a qualified specialist.
Demonstrate financial hardship or financial barriers that impact their ability to access the recommended treatment.
Provide accurate and complete information within the application process.
The Foundation may request additional documentation or clarification to verify eligibility.
3. Application Assessment Process
All applications are reviewed through a structured process that may include:
Verification of medical documentation
Review of financial hardship information
Consideration by an authorised Shebie Foundation grant review panel
Submitting an application does not guarantee funding.
Decisions are made based on available funds, eligibility criteria, and the Foundation’s mission.
All funding decisions made by The Shebie Foundation are final.
4. Use of Funds
If an application is approved, grant funding must be used solely for the approved medical purpose outlined in the application.
This may include:
Surgical gap fees
Specialist consultation costs
Hospital-related expenses associated with approved treatment
Funding may be paid:
Directly to a medical provider, or
As otherwise determined by the Foundation.
Grant recipients may be required to provide confirmation that funds were used for the approved purpose.
5. Medical Disclaimer
The Shebie Foundation does not provide medical advice.
The Foundation does not recommend, endorse, or guarantee the services of any medical practitioner, hospital, or healthcare provider.
Any medical treatment decisions remain solely between the applicant and their healthcare professionals.
Receiving financial assistance from The Shebie Foundation does not imply endorsement of any medical practitioner or treatment.
6. No Guarantee of Medical Outcomes
The Shebie Foundation provides financial assistance only and accepts no responsibility for medical outcomes, treatment results, complications, or healthcare decisions.
Applicants acknowledge that all medical treatment carries risks and that responsibility for treatment outcomes lies solely with the patient and their healthcare provider.
7. Accuracy of Information
Applicants must ensure that all information provided during the application process is:
Accurate
Complete
Not misleading
If false or misleading information is discovered, The Shebie Foundation reserves the right to:
Withdraw funding approval
Cancel an application
Recover funds where appropriate.
8. Confidentiality and Privacy
All personal and sensitive information provided as part of the application process will be handled in accordance with The Shebie Foundation Privacy Policy.
Health information will only be accessed by authorised individuals involved in the grant review process.
9. Limitation of Liability
To the extent permitted by law, The Shebie Foundation, its board members, volunteers, advisors, and partners are not liable for any direct or indirect loss, damage, injury, or medical outcome arising from:
Any treatment received by an applicant
Reliance on information provided during the grant process
The use of funds provided by the Foundation
10. Changes to the Grant Program
The Shebie Foundation reserves the right to:
Modify grant eligibility criteria
Adjust funding amounts
Pause or discontinue grant programs
at any time depending on available resources and operational considerations.
11. Acceptance of Terms
By submitting an application, applicants confirm that they:
Have read and understood these Terms & Conditions
Agree to comply with the application requirements
Acknowledge that funding decisions are made at the discretion of The Shebie Foundation.
